*You will need to reference the customer ID#W2525, know the year of your last visit and the last name you used as a client.

Patients can request a copy of their medical records for up to 6 years after their last visit. (For instance, a patient who last came to Aradia Women's Health Center in 2004 would be able to get a copy of her medical record until 2010.)

A fee of $2 per file, plus standard postage costs, will be required for each medical record request.
